Is there any additional fee apart from the $120 application fee. Can you guarentee the externship position in the desired place once we send our documents and the application fee?

And is the updated immunization documents must?..I understand that foreign medical graduates usually don't have health insurance in US,..so how will they have an updated immunization documents? Excuse me for my lack of knowledge in this area.
