I think the answer is C Intractable seizures as infant formula and breast milk are the only source of electrolytes for babies so if you add free water you are reducing the amount of sodium the baby is taken and causing hyponatremia which causes Intractable seizures which could only be treated when you increase the Na level to at least 120 (That is why it is intractable as anticonvulsant medications can not stop the seizures), all other choices don't make sense as a 3 week old baby won't c/o nausea and babies can get diarrhea from the formula when it is concentrated not diluted.
