Sample MSPE for IMG's - asc

Does anyone have a sample of the new MSPE that we can send to our med. school to be signed as they wont fill it out I know that for sure. I have the one on the ECFMG site but maybe someone did it already and can send me a word document sample. I will appreciate it.

asc ask ur med school clerks i'm sure they'll know how to make MSPE or Dean's letter ( they r 2 names for same thing)
