well, I am just thinking that a wedge shaped necrosis in the lung periphery is a pulmonary embolism. So I am guessing that a wedge shaped necrosis in the brain is also an embolic phenomenon.

C... As the patient has a history of massive MI recently, there is a very strong possibility of ventricular dysfunction arising as a result of it. This would cause Hypotension --> decreased cerebral perfusion and hypoxic neuronal injury.
