ok when i designate a new lor, what do i put in "title/department" field

do i put the medical center name?

title: professor, assisitant professor....
dept: the dept that the writer works in. u can add the name of the university. eg. Dept. of Pediatrics, UCSF.

ohh ok for example:

i put "physician" for the title and the association he is part of.

so i put both, "physician, the association of..."
