Hyperacute rejection is a type III reaction that occurs within 8 to 24 hours when the blood type (in the ABO system) is not matched and the expression of the incorrect antigen on the endothelial cells of the transplanted kidney promotes immune complex deposition and complement activation on the walls of the blood vessels. The rapid time course of the reaction indicates that a preformed response (antibody) is being activated.
