a 46 y/o smoker and alcoholic with abrupt onset of spiking fever, productive cough with blood-tinged mucoid sputum and pleural chest pain. PE reveals dullness to percussion, crepitant rales , and increased tactile fremitus in the right upper lobe. A CXR reveals a lobar consolidation in the right upper lobe. the G stain result would be ;

1. G+ rods
2. G- rods with capsule
3. G+i in clumps
4. G- diplococci
6. G+ filamentous bacteria

The most common cause of pneumonia in smokers and alcoholics are 1. pneumococcus (G+ coccs in diplo or short chains) and 2. Klebsiella (G- rods with capsule).
The best fit would be 2, I think.

2222 the keyword muciod bec klebsiella has very thick capsule causing muciod or currant jelly sputum. D.D pneumococcus has rusty sputum but gram +ve bug . and also #1 in alcoholics pts but sometimes they are looking for klebsiella. GL .
