my two cents...:

I think with staph aureus they would have mentioned that he's an IV drug user.and you're right, this question is incomplete..usually they do mention new murmur after dental/urological procedures/colonscopy and then give you choices.

3 weks low grade fever...........viridans...subacute onset

but hdoc how can one justify 3 weeks of illness will lead to valve damage
also hdoc what wud u opt for if the options has both aureus and viridans

murmur may b carditis murmur...not valvular....if it was not there prior
but with viridans....occurs on preexisting damaged valves usually....
wat answer have they given...plz tell
