its a hypermetabolic state causing high fever, tachycardia, and initially HTN followed by hypotension and CHF with decrease CO leading to AMS and delirium. In terms of management:
NBSIMg------> IVF(dextrose preferred)+ packs/cooling+acetaminophen
HTN and tachycardia----->propranalol
High dose PTU (inhibit peripheral T4 conversion to T3)
Glucocorticoids/potassium iodide/or iodide (also decrease T4 to T3 conversion)
Good luck
