NBME 1 A 25-year-old man with a history - khushigrover

19. A 25-year-old man with a history of intravenous drug use comes to the emergency department because of a progressive diffuse headache, generalized malaise, and low-grade fever for 2 months. During this period, he has had a poor appetite resulting in a 6.8-kg (15-lb) weight loss. His temperature is 38 C (100.4 F). Examination shows ¬¬neck stiffness. Mental status examination shows no abnormalities. Cranial nerve examination shows weakness of the lateral rectus muscle on the right and bilateral papilledema. A CT scan of the head with and without contrast shows moderate ventricular enlargement. Examination of cerebrospinal fluid shows:

Glucose 35 mg/dL
Protein 150 mg/dL
WBC 100/mm3
Lymphocytes 100%
RBC 1/mm

BUT HOW WOULD U EXPLAIN HYPERTENSIVE CHANGES...papilledema AND LATERAL RECTUS PALSY...........

Incr pressure can occur in both HSV or crypto
RBC must be > 10 for it to be HSV n Glucose is often nl or high
CT should reveal temporal lobe lesion

HEY KINDLY EXPLAIN HOW U CAME TO THIS CONCLUSION..BECOZ FA SAYS PRESENCE OF RBCS WITHOUT TRAUMA SUGGEST HSV

1]fungal meningitis has almost same lab profile as bacterial meningitis .. low glucose high protein but the main difference is instead of high neutrophils you will have high Lymphocytes .. here it says that myphocytes are very high .
2]in viral you wont have that low glucose and that high protein.. though the predominant cells will be lymphocytes
3] HSV encephalitis is bitemporal encephalitis , it has very specific bi temporal headaches so thats another characteristic
4] RBC value is not that high and it can be present in that amount in several infections so thats not very specific sign here to distinuguish
