Both.
The chief complication of heparin is hemmorhage.

Chronic or intermittent administration can lead to reduction in antithrombin III activity ,decreasing the inactivation of coagulation factors, and increasing the risk of thrombosis.Low dose heparin serves better.

Platelets are activated by IgG mediated reaction with heparin platelet factor 4 complex, causing paltelet aggregation, release of platelet contents, thrombocytopenia and thrombosis.Treat with Lepirudin.

one doubt genius..thr is a drug called danaparoid, idn`t that used before lepirudin??and argatroban comes in the same calss as lepirudin?

argatroban is also thrombin inhibitor and can be used in pts with HIT.both argatroban and Lepirudin has same efficacy, except that arga....has shorter half life so given with continuous infusion.

*And if pt has severe renal insufficiency then argatroban is preffered(argatroban has r so for renal pt)

*if pt has hepatic insuffieciency then Leperudin(L for Liver to remeber)

and Danaproid is anti-factor Xa,and Antithrombin inhibitor. It has been approved for DVT(esp in Hip surgery),can also be given in HIT (as anti factor X activity is very very more than antithrombin activity)but those with direct thrombin inhibitor activiy are preferred.not given in ts allegic to pork(as made from porcine mucosa)
