strange case of CCS - jeanbaptiste
#1
My friend who finished Step3 told me another case.

pt. is a male in his 60s.
temp: 39.6
HR: 110
BP: 100/60

before the pt came in, the temperature was hot for 2 days.

pt did not have any sign of infection in UA or blood culture or urine culture. but had a WBC count of 16,000.
pt's lytes was high sodium, high chloride, high Cr
(my friend don't remember further)
my friend first thought some infection and tried every test but no sign of infection.
but, he had his 5 minute window right after he put foley catheter and urine output and pt was somewhat better.

my friend just wrote "dehydration" for the diagnosis.
he did not remember similar case in UW.

So, what do you think about this case? (sorry for the insufficient information, that's all he told me)

hypernatremia can cause fever and increase in WBC count....I wonder if this is some of the endocrinopathies

If the window comes early with having any clue then he missed some thing........................
May be in the history. or examination.
This happens most when you dont read some thing in his or examination.

we believe that we read every thing but ............
So my frind take it easy.

I think heat exhaustion/stroke since you said 2 days hot..

If it was a heat exhaustion, then what do we have to do for the treatment ?

Do they have similar cases in UW CCS ?

Do they have some orders like 'cool blanket' or 'vaporization technique' in Primum CCS ?
