65y/o with crf has n/v.had these symptoms from 3 days.denises similar prior episode.no abd pain,dysphagia.no fever.PMH-DM,HTN.takes metoprolo &insulin.vital signs stable.lung,heart,abd exam normal.bun-50mg/dl..cr-7mg/dl
most appropraite step-
1)hemodialysis
2)xray abd
3)prescribe metoclopramide
4)prescribe ondansetron
5)schedule endoscopy

persistant N&V is a sign of uremia.so hemodialysis is certainly indicated.

in the absence of the electrolytes measurment I will consider this nausea and vomiting is a sign of uremia(mostly)so we have to==> dialysis
