On page 12 there is a qtn on STATE LICENSE ..

is it meant for IMG or only AMERICAN GRADUATES?

Almost all IMGs should go for NONE there, unless you already have ECFMG cert. and have gone through a post graduate training in the US and have applied for a licencse in a State before.
