1-If pt be in acute chest pain in ER,do we counsel him,regarding smoking,alcohol,...?
2-Do we ask sexual hx in sever headache like SAH?

Yes, we councel everyone on everything, but since we have acute chest pain in ER, we need to mention that it may be triggered, contributed by smoking etc. and you would like to talk bout it when you see this atient back in your office next week

Regarding sexual Hx, it depends if it is relevant to the case (memingitis -gonorrea, etc). If it is irrelevant, you may ommit it, but I still would recommend you to "touch" this subject somehow in a younger patient...
