I applied for IM Preliminary, Category and Primary, all in one program. I saw 3 different program codes in NRMP. During interview they asked me about my preference. I told them about my preference in order. Because they contacted one IV for all. Look like I need to enter all 3 codes in my ROL based on my preference, but still want to confirm with someone who have similar situation. Should I need to enter all 3 program codes into my ROL?

Yes...Enter all three codes. Absolutely

Since they have three separate codes you need to enter your in order of your preference.
