Could any one please tell me if a recommendation letter from the latest employer is required for the match purpose? For example, can I substitute it with one from the PhD advisor rather than one from the postdoc mentor?

I never seen such requirement, i would say it shouldn't be a problem, so don't worry.

No, it can be from anyone you like, but it is preferable that the letters are from medical doctors who have worked with you!!!
