a little gift back to this forum...my FM list - biggest_loser
#1
Hi guys,
i had posted the detailed psych list I had made by calling/emailing each and every program in US. Here is the link for it again.

http://www.usmleforum.com/showthread.php?tid=393177

Also, as I said, I made a list for Fm also.
However, i will like to add that as I was mainly interested only in Psych...my Fm list is little "raw" :-p

Also, as I have 4 attempts on my step2ck, I could not even apply to so many states.

So, please use it if it can be of any help to you.

Again, as I made it for personal use, it is kind of untidy :-p But, I do not want to go back and make it look pretty. :-p
Hope it will be fine.

Also, my creds are:
Step 1-- 78 , 1st att
Step 2ck -- 86, 4th att
Step 2cs --pass, 1st att
Step 3 -- not taken yet
Also, I need visa

So, please use this list acc to your req.

And, if there are few programs that have no details in front of them, it is because: they do not sponsor visa, or, i could not apply there because of my 4 att, or i just could not get any info about them despite trying again and again.

Good Luck to all!!!! Smile

Long Beach Memorial Medical Center Program
Long Beach, California 120-05-21-044
Dear International Graduate:

Long Beach Memorial Family Medicine accepts applications only through the Electronic Residency Application System (ERAS). To enroll with ERAS you need to contact the Association of American Medical Colleges (202) 828-0400. You must also be enrolled with the National Resident Matching Program (NRMP) (http://www.aamc.org/nrmp and be cleared by the Education Commission for Foreign Medical Graduates (ECFMG) 215-386-5900 or http://www.ecfmg.org

For graduates wishing to enter training in the State of California, you must first receive written clearance from the Medical Board of California. We will not interview applicants who do not have a œCalifornia letter. Contact them at 916-263-2486 or
http://www.medbd.ca.gov.

We are affiliated with the University of California Irvine and must comply with their rules regarding visa sponsorship. Foreign nationals may have a F-1 or H-1 Visa. UCI will sponsor the Visa if you match in one of their residency programs after one year. A foreign graduate who attended a foreign school must be a citizen of the US or a permanent resident.

Having met all of the above, the same criteria is applied as for US graduates. We review transcripts, letters of recommendation and USMLE results. We do not consider anyone who has failed the USMLE on any attempt, nor do we consider applicants with any below average grades on their transcripts. We have been fortunate to have several hundred applicants apply each year. We cannot interview every applicant. We only rank applicants that we interview.

Statistically, there are more unfilled positions in the mid-west and east coast than on the West Coast. We have always filled all our positions from the NRMP Match Program. The Association of American Medical Colleges has a website that we participate in that allows you to log on and find open residency positions. It is http://www.aamc.findaresident.org . You can also listen to the American Academy of Family Physician™s Hotline number 1-800-288-2668 regularly to learn where open FP residency positions exist. We used this hotline when we had an open position and also advertised the open slot on the AAFP™s website “ http://www.aafp.org.

Los Angeles, California 120-05-11-047
Dear Residency Applicant,

Thank you for your interest in the Southern California Kaiser Permanente Residency and Fellowship Programs. Kaiser Permanente has provided excellent medical training to thousands of medical students for over 50 years. Our objective is to educate and mentor medical students to develop the skills necessary to become accomplished and compassionate physicians.

The Kaiser residency and fellowship programs are very competitive and files are reviewed independently as well as comparative to the rest of the applicant pool. We accept and review files of all qualified applicants for our residency program as well as those on educational visas. We accept all visas, but are currently unable to sponsor due to the budget. There is no minimum board score requirement, however, multiple attempts can adversely affect your application as can many years away from medical school. Clinical experience is not required. We are not able to pre-screen applicants by e-mail or phone to determine whether they qualify for our program.

Residency applications can be submitted electronically through ERAS or AMSSM. Please check our website for details on which method to use.

The deadline to apply to most programs is Jan. 1st, however we may fill our interview slots before that date and it is best to apply as early as possible. A detailed description of the curriculum as well as program application guidelines is available on our website at http://residency.kp.org/scal.

Candidates interested in participating in a clerkship, or elective, can check the website for requirements and instructions on how to apply. We currently do not offer observerships nor externships.

Congratulations on the next step to reaching your educational goals. We wish you the best in the match for this year.

Sincerely,

Generally speaking applicants considered for interviews should have graduated within the last five years and have USMLE scores of 80 or above with not more than one failure on any exam. In addition, IMG applicants must be ECFMG certified and have a Post Graduate Training Authorization Letter from the California Medical Board before being considered for an interview. We do not sponsor visas but do accept J1 visas sponsored by the ECFMG. We will be accepting applications, only through ERAS, from September 1 until December 1.

Academic Year 2008-2009

Dear Applicant:

Thank you for your interest in our Family Medicine Residency Programs. We no longer send out residency brochures or applications. The Department of Family Medicine has a main web site at http://fammed.uchsc.edu on which you can review our application process, eligibility and the links to our family medicine programs, each detailing its own unique faculty and curriculum:

Rose Medical Center (http://www.rfmr.com)
Swedish Medical Center (http://www.sfmr.com)
University of Colorado Hospital (http://fammed.uchsc.edu/uh) and
Denver Health (http://fammed.uchsc.edu/dh)

Our programs use the Electronic Residency Application Service (ERAS®). You will be able to get this information from your medical school. We do not accept any paper applications.

The application process is the same for all four programs, however each program determines its own eligibility criteria. Please review each program website to determine if you meet program requirements. Please submit an ERAS® application, a personal statement, a Dean™s letter, transcript, USMLE scores, and three letters of reference. Among the letters of reference you must include at least one from a Family Medicine physician who has supervised you clinically.

Please incorporate into your personal statement your answers to the following questions:

1. How did you become interested in Family Medicine?
2. What is it about Family Medicine that appeals to you?
3. What other experiences or interests played an integral part in your life?
4. What type of practice do you plan on pursing after residency training, and what type of patient population would you like to care for?

Our residencies are very competitive and interviewing time is limited, therefore, we suggest getting your ERAS® application in early. Again thank you for your interest in our programs.

Emory University Program
Atlanta, Georgia 120-12-21-562
Application Requirements/Selection Process

Applications to the Emory Family Practice Residency Program for 2009-2010 will be accepted only through the Electronic Residency Application Service (ERAS). This service is available to all US medical/osteopathic graduates through medical school dean™s office. Canadian medical school graduates should contact the Canadian Resident Matching Service for ERAS. International medical graduates may access ERAS through the Education Commission for Foreign Medical Graduates (ECFMG).

A completed ERAS application must include a USMLE or COMLEX transcript. International medical graduates must also ensure that their ERAS application includes an ECFMG certificate (including a current TOEFL) and a copy of a translated medical school diploma. All candidates should have taken USMLE Part I and are strongly encouraged to take Part II and forward their scores to the Emory Family Medicine Residency Program prior to February, when our ranking list is finalized and submitted to the NRMP.

First-year positions are offered through the National Residency Matching Program, and recruiting is in compliance with the NRMP guidelines.

Second-year applicants or applicants with a minimum of 10 months of transferable advanced placement credit (per ABFP guidelines and application) will be considered if openings occur in this level. These PGY “2 applicants will be interviewed and considered outside the NRMP.

Selection Policy

Academic records, letters of recommendations (at least one of which must come from a family physician), USMLE (or FLEX, NBME, COMLEX) scores, honors and applicants personal statement and future plans are considered in choosing to interview an applicant. The interview, compatibility of the applicant with program philosophy and goals, academic record, letters of recommendation, and evidence of involvement in and commitment to family practice weigh in the acceptance and ranking of applicants.

International Medical Graduates (IMG) Policies
All IMG candidates must have documented clinical experience in the US or Canada, and the majority of recommendations must come from physicians that can attest to this clinical experience per Emory™s Graduate Medical Education guidelines. Only those IMG™s with an H-1 or J-1 visa or permanent resident status may be enrolled in post-graduate medical training. Emory will sponsor the H-1 and J-1 visas providing the IMG have documented evidence of successful completion of all necessary components by December 31, 2007. This includes passing scores for USMLE Steps 1, 2 and 3. Application will be withdrawn from consideration if this requirement is not met.

Emory School of Medicine and Department of Family and Preventive Medicine does not discriminate in recruiting on the basis of race, national origin, color, religion, gender, sexual orientation, age, veteran™s status, or disability. We value diversity and individuality in our candidates.

Deadlines for Application
All candidates must submit applications through the ERAS. Applications are reviewed periodically, and candidates are notified when we have sufficient information to grant an interview. Interview dates are granted on a rolling basis to qualified candidates. The application deadline is November 30, 2008. Interviews will be scheduled on selected Mondays and Fridays from October 1 through January 31, 2009. The interviews last the entire day. With enough advanced notice we may be able to arrange overnight lodging and dinner with one or more of our residents. Spouses and significant others are invited to visit with us as well.

The Medical Center Program (Columbus)
Columbus, Georgia 120-12-11-092
Thank you for your interest in the Graduate Medical Education Programs sponsored by The Medical Center in Columbus, Georgia. The Department of Medical Education sponsors a three year fully accredited Family Practice Residency Program in addition to a one year Transitional Year program.
Please visit our Web site for more detailed information about our program.
The URL is www.columbusregional.com/familypracticeresidency.
We hope that you will find the material informative.

Our program accepts all residents through the NRMP match. Our residency programs accept applicant materials through the Electronic Residency Application Service (ERAS) available at all US Medical schools and the office of the ECFMG. Our program will review applications in an ongoing basis as they become available on the ERAS system.

Our programs consider and accept International Medical Graduates. There are many IMG graduates of our program successfully practicing in the Columbus area and throughout the country. Successful applicants will have had strong scores on Step 1 and Step 2 of the USMLE. We do not screen applicants by cutoff scores alone. Most of our successful applicants will have had at least one year of clinical experience in the American healthcare system to be competitive. Our program requires successful ECFMG certification. Most of our successful International Medical Graduates have graduated from their medical schools within the past 5 years. Unfortunately, our hospital system does not sponsor J-1 or H-1 visas. All successful applicants must have permanent residency certification (green card) in order to be considered.

Thank you again for your interest in our program if your application meets the above criteria we would be most interested in hearing from you. Please forward either a letter summarizing your qualifications or send an email to john.bucholtz@crhs.net

Sincerely,

John R. Bucholtz, D.O.

Idaho State University Program
Pocatello, Idaho 120-15-21-521
Dear:

Thank you for your interest in the Idaho State University Family Medicine Residency Program. All application materials must come through ERAS before the deadline date of December 1st. In order to be considered for an interview you must have graduated from a medical school that is licensable in the State of Idaho. You may call the Idaho State Board of Medicine to obtain this information. We would prefer that you have been in medical school within five years of your application date. Also, you must be certified by the ECFMG if you are a foreign medical graduate.

Exams
It is difficult to get an Idaho Medical license if you have failed any exam twice. However, any passing score is acceptable. You must have passed the USMLE Part I and both the clinical knowledge and the clinical skills exams of Part II. The program expects the equivalent usual clinical experience achieved in a U.S. medical school.

Visas
It is very unlikely that we would be able to sponsor visas. Most of the foreign medical graduates in this program have permanent US resident status or green cards. Generally speaking, we accept approximately one foreign medical graduate every other year.

For more information, please visit our website at www.fmed.isu.edu . Good luck in your search process.

Sincerely,

Lisa Redford-Prince, LSW/MPA

MacNeal Hospital Program
Berwyn, Illinois 120-16-11-098
Dear Prospective Applicant:

Thank you for your interest in the MacNeal Family Medicine Residency Program. At this time I™d like to inform you that our Program will only be accepting applications through the Electronic Residency Application Service (ERAS). Please see our Web Page at www.macneal.comfp for more information regarding our Program.

As one of the first family medicine residency programs in the country, and the first in Illinois, we strive for excellence in education and patient care.

MacNeal Hospital is an equal opportunity employer. The Family Medicine Residency will not discriminate with regard to sex, sexual preference, race, age, religion, color, national origin, disability or veteran status.

We receive approximately 450 applications each year to fill our twelve positions. All applications received prior to the deadline (usually the first week of February) are reviewed. All applicants are evaluated based on preparedness, aptitude, ability, academic credentials, communication skills, and personal qualities.

Factors that weigh into the decision to offer an interview include:

¢ Letters of recommendations, especially those specific to a recommendation for Family Medicine.

¢ Individuals whose application reflects someone specifically seeking family medicine training.

¢ USMLE scores are reviewed, and in addition to numeric scores the number of attempts on each exam considered as well

¢ Strength of Medical School

¢ Individuals who have graduated from medical school within the last two
years

¢ At least one year of clinical experience in the United States™ health system, preferably as part of medical student clinical experience

¢ Please note we do not sponsor Visas at this time nor do we have plans to do so

University of Louisville Program
Louisville, Kentucky 120-20-21-145
Congratulations on your interest in Family Medicine! We are very pleased that you have expressed this interest and confident that you will find Family Medicine to be a rewarding career. The University of Louisville and our community partner, Jewish Hospital and St. Mary™s HealthCare, are working together to provide an exceptional training experience for our Family Medicine residents. A resident in the Department of Family and Geriatric Medicine receives the best of both training worlds: experiencing incomparable tertiary experiences in the university setting during the PGY-1 year and expanding out into the community for PGY-2 & 3 rotations. Our residents have two community Family Medicine Center options for their continuity experience. Fellowships in Sports Medicine, Geriatrics and Palliative Care are also available.

We will consider graduates of U.S. and international medical schools for first year level positions (PGY-1) through the National Resident Matching Program (NRMP) if the individual has one of the following:

1. Prior or anticipated graduation from a LCME accredited medical school.
2. Prior or anticipated completion of a "fifth pathway" program.
3. Graduates of medical schools outside the United States and Canada who have not completed a œfifth pathway program must have current, valid certification from the Educational Commission for Foreign Medical Graduates (ECFMG) at the time of application. Current students in such schools must have ECFMG certification to begin residency.
and:
1. International medical school graduates must have completed twelve months or more of clinical training (i.e. observership/externship involving patient contact) in an American or Canadian medical setting within the last two years.
2. Applicants must be a U.S. Citizen, have permanent residency status in the United States or be eligible for a J-1 Visa.
3. Applicants must have a passing score (on the first or second attempt) on USMLE-1 and both parts of USMLE-2 (Clinical Knowledge and Clinical Skills) or other recognized licensing examinations. Applicants with successful completion of USMLE-3 will be given preferential consideration.
4. Applicants who have graduated from medical school within the last five years will be given preferential consideration.

The Family Medicine Residency Training program at the University of Louisville will only accept and review applications submitted through the ERAS system. If you are an international medical graduate, you may contact the Educational Commission for Foreign Medical Graduates for more information on the requirements for ERAS. If you are a U.S. medical graduate, please contact your school™s Student Affairs Office for assistance with the ERAS process.

For general residency requirements, you may access ACGME™s website at www.acgme.org. For specific information regarding our Department of Family and Geriatric Medicine, you may access www.familymedicine.louisville.edu.

December 1 is the deadline for applications. Applications received by November 1 will be given preferential consideration.

Invitations to interview may be offered after review of the above documents. No invitations will be issued unless the application is complete within the ERAS system. In keeping with the University of Louisville School of Medicine™s Mission Statement, the Department of Family and Geriatric Medicine will give preference to applicants with ties to the Commonwealth of Kentucky.

Is there a limit on the number of attempts passing the steps? No
Is there any cut off point for the year of graduation? Must have graduated within the

The Family Practice Residency Program at The Brooklyn Hospital Center is a unique, progressive and innovative program. The Program™s central goal is to deliver high quality, culturally-appropriate health care to the diverse urban population base that we serve and to advance the family-oriented healthcare model in the community.

Over the course of three years, residents are exposed to a broad spectrum of clinical medicine rotations which provide a myriad of medical, psychological and social problems that extend beyond the individual. The Family Practice Inpatient Service similarly provides a multi-disciplinary approach to hospitalized patients, under the supervision of Family Physicians as well as subspecialists in other disciplines throughout the Hospital. Residents can further strengthen their knowledge base through conferences and curriculum designed to shape the proficient Family Physician.

The Program is fully approved by the Accreditation Council for Graduate Medical Education and prepares residents for certification by the American Board of Family Practice. The program is approved for 15 positions--5 in each year of training.

Thank you for inquiring about the University of Rochester/Highland Hospital Family Medicine Residency Program.

All PGY1 positions beginning in July 2008 have been filled. Should a position become available, it will be posted on Find A Resident and applications posted on Find A Resident will be reviewed. Due to the high volume of inquiries, we are unable to save email for later review.

You are welcome to visit our website at http://www.urmc.rochester.edu/smd/gme/pr..._medicine/ for more information.

We accept international medical graduates for the University of Rochester Family Medicine Residency Program. We consider applicants who have passed Parts 1 and 2 of the USMLE on their first attempt and also have clinical experience working as a physician in the US or Canada. At minimum, this should include a significant US experience as part of your formal medical school curriculum or some other formal professional period such as a transitional or preliminary year of residency. Externships, observerships, and research are usually not considered enough experience to meet this requirement. Graduation from medical school within the last 5-7 years, with continued primary care experience during any intervening years, is preferred.

We are able to support J1 visas.

You are welcome to apply through the Electronic Residency Application System (ERAS) for a position that would begin June 2009.

If you are currently enrolled in medical school and would like to participate in a clerkship, please contact us for further information. We can only offer such experiences to medical students currently enrolled in a participating medical school which has a formal agreement with the University of Rochester School of Medicine and Dentistry.

We are unable to offer experiences such as observerships or externships to those not currently enrolled in a participating medical school.

If interested, volunteer and employment opportunities at the medical center are posted at http://www.stronghealth.com/about/fos/volunteer.cfm and http://www.stronghealth.com/employment.cfm.

Again, thank you for considering our program.

Dear Doctor,

Thank you for your interest in Flower Hospital™s Family Medicine Residency Program. Flower Hospital is located in Northwest Ohio a suburb of Toledo, Ohio and one hour south of Detroit, Michigan. Flower Hospital's Family Medicine Residency Program was the first Family Practice Residency Program established in Ohio and continues to educate excellent and caring family physicians.

We are accepting applications for the 2008 -2009 academic year only through ERAS. Please insure your application is complete to expedite its review. While there is NO MINIMUM score, applications are highly competitive. We have a history of training international graduates but we do not sponsor any visas. You can not have had more than 5 tries on the USMLE (total) and must be ECFMG certified by January 2008. We require one year of clinical experience in the United States for international medical graduates, which does not include research. Our hospital does not have observer ships or externships.

Under special circumstances we would consider an applicant who graduated from medical school more than 5 years ago, but our cut off year of graduation is 2003. Furthermore, due to stipulations by the Ohio State Medical Board, we will NOT consider applications of individuals who passed Step 1 or Step 2 of the USMLE more than 6 years ago.

All of the requested information must be received before your application will be reviewed. Our Admissions Committee will review completed applications during the months of October and November and will notify applications of their status via email.

The following items must be submitted for your file to be reviewed by our Admissions Committee:

Completed ERAS application form
Three to four letters of recommendation from:
1. Your medical school
2. Training experiences abroad
3. 1 year Training in the United States
4. Faculty member in training program (if applicable).

Dear Applicant:

I am asked frequently about our policies for selecting applicants. We have the following selection criteria:
1. With in 5-6 years of medical school graduation unless you have had recent clinical experience
2. We would like US experience but not absolutely necessary. We have several residents without US experience
3. Step exam 1, 2 & CS requires a pass (75) but obviously the higher the better and its better to pass on the first attempt than 2 or 3 attempts

We do accept International Medicine Graduates in our program. In our residency we have approximately 60-80 % IMG™s depending on the PGY 1 match. We sponsor J1 and H1b visas. However the expense will not be reimbursed to you. H1b and J1 visas should be applied for immediately on notification of matching with us. We highly recommend the utilization of a lawyer specializing in Immigration. This assures a start date of July 1 each year.

We accept only applications through the ERAS match system. Each application is review by our recruitment team who will decide on applicants interviewed. We interview approximately 70-75 applicants each year. You will be interviewed by a senior resident, Associate Director, one additional faculty member and I. We will be starting interviews by Mid October, 2008

We do not have any open PGY 1 positions available for this year (2008-2009)

University of Pittsburgh Medical Center Medical Education (Mercy) Program
Pittsburgh, Pennsylvania 120-41-11-578
Dear Dr./Sir/Madam:

Thank you for your request for information regarding the UPMC Mercy Family Medicine Residency Program. We have received many inquiries regarding our Residency Program requirements. I have listed below answers to the most often asked questions.

1) All applications must be submitted via the Electronic Residency
Application Service (ERAS).

2) We are looking for applicants who have graduated from medical school
within the past five years, but all applications will be reviewed.

3) One year of residency training in the United States is preferred,
but not mandatory. Any U.S. training will be taken into consideration.

4) You must have a valid ECFMG certificate prior to the Rank Order
List deadline.

5) You must pass the USMLE or equivalent on the first attempt, or
achieve a minimum passing score of 80 or above.

6) UPMC Mercy prefers the J-1 Exchange Visitor visa, but recognizes
that you may prefer a different type. Mercy will cooperate with
you to obtain the desired visa, but the burden of obtaining the visa
and all related expenses will be up to you.

7) The application deadline is January 15.

We are sorry, but due to the limited number of positions we have available, and the large quantity of applications we are receiving, we are not able to offer all applicants who meet this criteria an interview.

I hope that this answers all of your questions. Should you have any further questions, or need additional information, please feel free to visit our website at www.pmhs.org/gme/meded/familypractice/ or contact me at 412-232-5955 or 1-800-978-6661.

Dear
Thank you for considering the Family Medicine Residency Program at Memorial Hospital and Brown University. The requirements for applying to our residency program are as follows: You must apply through ERAS. Once you apply we will need a completed application form, personal statement, 3 letters of reference, Dean's letter, transcripts and the scores from the USMLE. All of this information must be sent electronically. You must have passed USMLE Steps 1 and 2 by the second try, with a score of 200 or better on each Step, or have successfully completed two years in a LCGME accredited medical school. Two positive evaluations of the experience(s) must be submitted. SELECTION OF RESIDENTS

STATEMENT OF POLICY: Residents are selected on a fair and equal basis without regard to race, color, religion, sex, or national origin. Selection is based upon aptitude, academic credentials, personal characteristics, and ability to communicate verbally and in writing.

PROCEDURES:

1. All programs participate in the National Residency Matching Program (NRMP). Only applicants with qualifications as required by the Accreditation Council for Graduate Medical Education are considered eligible for residency. We accept applications from students from LCGME licensed medical schools and international applications if ECFMG certified. All applications must be through the AAMC ERAS application service. Applications from applicants that have been out of medical school for 4 years or more are not looked upon favorably.

2. All applicants are required to complete an application form and submit this along with board scores, letters of reference, a dean™s letter, and a medical school transcript.

3. The residency program compiles the data and prepares a personal record on each applicant. The program director and faculty screen applications according to established departmental criteria and selects applicants for interview. Applications are screened first by board scores. We want to see 80 and 80 as USMLE Part I & II scores as the minimum for consideration. The program prefers NO failed attempts of the USMLE exam. Multiple failures of a USMLE exam prior to passing are not looked upon favorably and you cannot be licensed in this state with more than four attempts of any stage of the exam. If an IMG, clinical experience in the United States is required.

4. A personal interview is granted to those applicants selected through the screening process. During this interview applicants are informed of salary, vacation time, professional leave, parental leave, sick leave, professional liability insurance, hospital and insurance benefits, call rooms, meals, laundry, etc. A copy of this is on the residency program web page at www.spartfam.org

5. At the end if the interviewing period, the Resident Selection Committee objectively evaluates each candidate and prepares a list of applicants in rank order, which is then entered for the National Match process within the Department of Medical Education.

6. We will assist the IMG by working with their attorney to achieve J-1 visa status. H1-B visas already in hand are preferred.
