A 27-year-old woman with insulin-dependent diabete - runnin17
#1
A 27-year-old woman with insulin-dependent diabetes comes to the clinic seeking help with her
medical regimen. Although she feels well and has no current complaints about her state of
health, she is having difficulty complying with her insulin schedule. She takes subcutaneous
insulin before meals and before bedtime. Aside from her insulin, she takes no other
medications. With her current busy schedule and her understandable dislike of self-injections,
she occasionally misses a dose of insulin. She has applied for an insulin pump, but
unfortunately her insurance company does not provide coverage for such devices. Physical
examination reveals a thin, healthy appearing young woman. Routine laboratory studies are
unremarkable. Which of the following is an appropriate adjustment in this patientâ„¢s insulin
schedule?
A. Change to insulin glargine, once daily before bedtime
B. Discontinue the insulin injection before bedtime
C. Recommend inhaled short-acting insulin formulations
D. Start metformin and glyburide, titrate off insulin
E. Use once-daily insulin lispro, inject before breakfast

C ???

coz she will need short acting insulin before meals! and that might keep her on track with her meds since she dislike the subcutaneous insulin?? I'm totally not sure about this!
