i am sick now so cannot prepare so want extent eligibility period. i want to know how and how long can i extent with this application.

You can go to your ecfmg account and extend ur triplet by paying a fee(i think it was 50 dollars).

I believe your period will be extended by three months. That is, if your triplet ends in april, then after extension you will get may-june-july.
