A 25-year-old G2P0 Caucasian woman - ritonavir
#1
A 25-year-old G2P0 Caucasian woman comes to the office for a prenatal visit. She is 11-weeks pregnant. She had an induced abortion six years ago. She denies the use of tobacco, alcohol or illicit drugs. She takes 800 micrograms of folic acid and a multivitamin supplement daily. A recently ordered abdominal sonogram shows normal placental maturation and an adequate size of the embryo for gestational age. There is a 7 cm left adnexal cyst present in the sonogram, which was previously identified in an ultrasound evaluation during her 7th week of pregnancy. Which of the following is the most appropriate strategy to prevent any complications?

A.

Remove the cyst surgically if it does not regress postpartum

B.

Remove the cyst surgically during the third trimester

C.

Remove the cyst surgically immediately

D.

Remove the cyst surgically during the second trimester

E.

Remove the cyst surgically by C-section when pregnancy is at term

D. Second trimester is the appropriate time. It reduces the risk of spontaneous abortion and gives time for the regression. Very tough question, in my opinion.
