For the arterial embolism, it depends on the site but generally we will give heparin then perform embolectomy.

true for PE depends...so say pt. comes in with SOB into ER and suspected PE...whats management

O2, ASA

Anticoagulation with Heparin or LMWH

Thrombolytics....make sure no complications to thrombolytics

For PE initial: ABG and CXR (the CXR is to exclude other causes of SOB not to dx PE)
ABG will show resp alkalosis
Start heparin
If the CXR is clear do V/Q scan if not do CT angio.
For bedridden patients skip the V/Q and the CT and do a lower venous U/S.
If all are negative or inconclusive do Pul angio.

For unstable patient use thrombolytics or embolectomy
For stable and heparin is contraindicated interrupt IVC
