exactly when u start oral feeding in DKA? - northeast

when bs reach 250 start s/q insuline with iv insuline, then after 1 to 2 hour of s/q insuline with no nausea and vomiting start oral diet and also discontinue iv insuline also

1.DKA=high bs,ketosis,acidic ph,low hco3------ivf=NSS+iv insulin[0.1 unit/kg/hour] regular/rapid acting,iv kcl once urine passed and k is less than upper normal limit,npo if abdo pain,altered sensorium or vomiting.


2.when bs=250 ,lower insulin infusion pump rate----dont stop insulin .....start iv dextrose 5% at 80 ml/hr....balance insulin with dextrose to maintain BS of 15 or so.

3.as soon as patient is stable and in a condiotion to accept oral feeds......no vomit,not drowsy...we can start oral feeds ...oral feeds also give us chance to shift patient onto s/c insulin
