PCWP is really HOT, as some folks described in the their exam experiences. I think this is well written by one forum friend. Since there is not copyright issues, I am inserting his note.
Again understand PCWP well. How to Dx shock? 1-Look at CO, if it is 5L (hyperdynamic), look at fever, if there is fever its Septic, if there is no fever, its Neurogenic Shock(or Anaphylactic shock. Bee sting,medication, food alergy).

understand the state of CO, PCWP and PVR in four types of shock.
There is a good chart in FA (pg 384; 2007 edition; types of shock) Tx modalities are different; hypovolemic--fluid resuscitaion; cardiogenic--dobutamine--1st choice or dopamine/norepinephrine; Septic---antibiotic and fluid; and Anaphylactic--diphenhydramin or norepinephrine

65 yr old diabetic pt come to routine 6-monthly checkup. He says he was a professional athelete before but is not able to run 10 miles everyday anymore so he stays at home most of the time. he takes sulfonylureas for his diabetes and his HbA1c is 8.9%. He also has Hypertension and he takes thiazide diuretic regularly. His Total cholestrol is 198 and LDL is 161. Wat is the next step in management of this pt's condition?
a) lifestyle change with regular exercise. b) furosemide to control hypertension c) glyburide d) Check cholestrol levels again after one month e) Start him on statins and lifestlye change. f) Start him on Gemfibrozil and Statins.

pt completed chidhood vaccination series already received 3 doses of TT,
treat as >3doses schedule, no TIG for this patient and TT if last vaccination >10 year and for clean wound, >5 year for dirty wound

What is bite cell and Heinz bodies? Why G6PD def associated with hemoglobinuria.
Denatured hemoglobin precipitates in the form of Heinz bodies due to oxidative stress. These precipitates are removed by splenic macrophages causing a distinct configuration observed on this blood smear(bite cells). These "pitted" cells are then prone to leak hemoglobin in circulation, which explains the hemoglobinuria -- a hallmark of the disease.
