What do u guys think....as the answer in key is adenoCa But i thought its Tubular adenoma which can also have all these manifestations if has a hemmorrhagic surface.......???

this pt is 56(>50) and has iron-def anemia, the Q is asking for most likely Dx, It should be adenocarcinoma

yes, I think it is adenoCA as well, occult bld, age and the size of the adenoma all point to that.

Oh my God!!!

look at the link anu & also adenoCa form ulcers, this pic looks like adenoma more
with a stalk & hemmorrhagic srf ----> can lead to occult bld etc.

what do u think now??

the pic from Upath looks more like the pic in NBME Q stem, with a hemorrhagic appearance and a bigger size, remember both point to CA in addtion to the pt's age and occult blood.
