1. failing heart: glycolysis as major source of energy?

3. Just remember GLT2 in hepatic cells and beta cells of pancreas, GLT4 in muscle and adipose cells

4. RBC always use glucose, nomatter when.
Resting muscle use glucose in fed state, use ketone in starvation
excercising muscle: break down glycogen

Yes,
Fetal heart uses Glucose primarily
Post Natal it switches to beta oxidation of Fa's as seen also in low intensity excersice after Glycogen stores are used up
Failing heart uses primarily Gucose and some beta oxid of Fa's
