As is well-known, each year the Residency Match becomes more and more competitive. Residency programs set application guidelines for their applicants, which include 1-scores, 2- visa status, 3-year of graduation,4- Documentation 5- US Clinical Experience. Many of these factors are predetermined and cannot be altered, e.g., one cannot change scores or year of graduation. However, you can still improve your application with strong US letters of recommendation from teaching attendings with inpatient rotations, and doing so can dramatically strengthen your application. Strong US clinical experience will not only enhance your application, but also acclimate you to the US clinical environment prior to beginning your residency. The first goal is meet the requirements for your application, the second equally important point it to make your transition from your ECFMG certificate to US medical practice smooth.

Unfortunately, residency programs are noticing a very unusual rise in CS exam failures which has been increasing in the past 2 years. A CS exam failure coupled with a lack of hands-on clinical experience will often keep an applicant from obtaining interviews and matching into a U.S. Residency program. The CS exam is a practical exam and preparing for the CS exam requires a different approach. Although you may be technically knowledgeable, nothing can possibly replace hands-on clinical experience in helping to acclimatize an IMG to the U.S. clinical system. In order to improve your chances you may utilize your Letters of recommendation to discuss your medical ability to counteract your CS failure.
