what is the Rx for ITP, TTP, myasthenic crisis,.....


is it prednisone,plasmaphresis, plasmaphresis, or IVIG respectively????

for ITP.prednisone...splenectomy if plt count

ITP..we usually advice reassurance if pt count is less than 50000 we advice steriods

TTP..yeah plamaphresis

myasthenic crisis..yeap plamaphresis or ivig

for ITP: prednosine, splenectomy is plt count

for ITP: prednosine, splenectomy is plt count is less than 10,000,
children with ITP--observe
for TTP: plasmapheresis if servere, obserivation if mild (Dipyramidole may help)
for MG crisis: plasmapheresis with IVIGs

And for MG crisis we have to stop any ChEI and intubate the patient, as he might have acute respiratory failure due to extreme muscle weakness.

Rx for ITP......adults...ITP.....asymptomatic no rx, symptomatic or count less than 10 to 20k........steroids for short periods of time.......refractory disease.....next rx spleenectomy as only rx that offers long term benefit.........if still refractory......then use immunosupressive drugs (rituximab, cyclophospamide, azathioprine), danazol is also an option for chronic refractory itp

if life threatening bleeding.........then IVIG.......rhogam if Rh positive patient.......though IVIG prefered in this setting.....once IVIG is administered can also give platelets in life threatening bleed

if kids.......usually observation best.......if symptomatic then IVIG prefered over steroids as side effects......spleenectomy last resort

TTP............plasmapharesis rx of choice

myasthenic crisis........intubate if VC less than 15-20 ml/kg or severe respiratory distress, stop Anti AChe inhibitors to decrease secretions...... plasmapharesis prefered over IVIG as more rapid.....start steroids at the same time.......
