1)others are positive family history of early CHD less than 65yrs for men and greater than 65yrs for males
2)age greater than or equal to 45 for men and women greater than or equal to 55
3)HDL less than 40
4)Hx of smoking
5)HBP greater than 140/90 or antihypertensive medications
