Here is my cred, 90/91 CS passed all first attempt, need visa, 2005 grad. research experience back in home country, 2 month USCE ( 1 in private clinic, 1 in non IM department ) 2 US lor..very strong ,2 from home country.

1 Work as a medical assistance in a private clinic. One of my Lor writer offers me to work in his clinic. He will process work permit for me.
2 Do research at NIH
3 Get USCE and help in some reserch in top notch university ( not IMG friendly one)
4. Try get other USCE in cmmunity hospital ( I don't know where yet... I do not know anyone )
5 Get Canadian clinical experience+ research with CA Lors---will it help at all? I have this offer from my teacher who is attending in top notch CA university. ( he did fellowship in US but does not have strong contact in US )

Any advice is greatly appreciated.

There are many observerships opportunities, check my site.
Get more LORs.
Don't limit yourself to northeastern area as most IMGs do this mistake, apply to multiple specialties (IM, FM, psych, ped) in DIFFERENT states.
Canadian experience is considered same as any foreign experience.

ataxicgaitsgirl: this is my opinion,

1. having an extra US LOR will do you good. if you want to go to NIH and I guess its the Bethesda one you are talking about; then find a job at building #10 - clinical

2. Option #3 is also a desirable alternative; but getting an externship in such places can be very difficult. Research in basic science will not do a lot of good for your application.

3. complete your step3 well before the application season begins.

4. where are you in the northeast? Eg: Sinai Hospital of Baltimore offers externship opportunities.

5. Keep your option #1 as the last option. In case the labor certificate or the application for some reason is rejected, you will have subsequent headaches when applying for visa. you have to do this move carefully if this is what you plan to take.

If you need money, this is the best option and convert this option to option 4, if the physician is also an attending in one of the teaching hospital.

2 Do research at NIH: research do not help in getting IVs in big way. waste of time and money.

3 Get USCE and help in some reserch in top notch university ( not IMG friendly one)> Absolutely preferred option over all other, if you are getting genuine USCE and not just research.

4. Try get other USCE in cmmunity hospital ( I don't know where yet... I do not know anyone ). Read answer for option 1.

5 Get Canadian clinical experience+ research with CA Lors---will it help at all? I have this offer from my teacher who is attending in top notch CA university. ( he did fellowship in US but does not have strong contact in US ): I don't know how it helps but not a bad option, if you are not getting anything in US.

What ever you do, do it once you finish your step 3. Step 3 can be finished in maximum of two months reading full time.

pyrimidine: how are you doing? are you back?

we seem to have contrary views about research; though I would agree that your view should take precedence over mine. though my point was that at bethesda the kind of clinical research done is of pretty high standard and you could also get patient interaction. that i thought was difficult to get opportunity.

Hi imgspouse, Congratulation to you and your spouse. I am happy that your IMG spouse has matched. I am still in India, taking care of my mom.

Look at my CV. I have 27 research articles as co author but none has asked me detail about those research articles except at one community program. With these amount of research work and with excellent scores including step 3, creating an atlas, I would have fetched more than 15 IVs, right? That is what I was expecting. But no that was not true. I could get only 9 IVs and only ONE university program.

And many programs here clearly mentions that research is NOT considered as USCE and will NOT be looked upon favorably. Of course, getting research along with genuine USCE will be the best option. But I would advice against JUST research.

You tell me what one can do in research here in US? Collecting data and feeding into a computer? Thats it. Most of the day they will sit in the corner with computer. They will not even learn how to write article, abstract, paper or even statistics b'coz they have specialist to do that work. Majority of time will be spent on collecting data for some one else which even a plain laborer can do.

What we need to learn is how US health care system is, how to manage common US medical issues, how to communicate with patients, history taking, bedside manners, etc which can only be learned through genuine USCE. Research will provide your CV with some good looking lines and your application will look little better. But if you want to improve your overall personality and want to boost your confidence, USCE is the better option.

and yeah I agree that some of the higher institute like bethesda, UCLA, johns hopkins, cleveland clinic or even cedar sinai are good options to be considered. But research involves high amount of uncertainty. You work your skin out in collecting data and what if your paper is not published or your name is not included. You can't do anything. You can not tell PD that you were involved in blah blah blah research at johns hopkins but the paper is nit yet published or was not accepted or your name was not included. for research PD needs your name in the journal article.

well these are my views on research and I might be wrong.

Hmm, see your point pyrimidine, and agree that research in general is not good enough or is considered USCE. But regarding the kind of research done, i think also depends on the supervisor and his/her interest as well as the candidate's aptitude... look at yourself.

Plus your last sentence is absolutely correct, and these days hands on experience is the only USCE folks should try to get.
