When do we do surgical debridment in a patient with osteomyelitis?

when not responding to medical therapy

If ESR is still elevated after 4- 6 weeks of therapy, further treatment and possible surgical debridment is necessary

Should we wait for the bone biopsy in order to guive Antibiotics or should we start with empiric treatment..?

ESR is the only thing that we can use to follow the response in osteomyelitis...What antibiotics should we use?
