I had 3 LOR from anesthesiology observership from Cleveland Clinics, 1 LOR externship in private clinic for FM, 1 LOR for externship in cardiology in private clinic. For all of them I got three letters each that individually stated my interest in different specialties. So For example, the cardiologist gave me 3 identical letters each with a different last line stating my interest in IM For one, FM for the other and pediatrics in the third as a back up. I also got one general letter from each doctor that only Specified my interest in medicine in general do it was easy to upload letters.

I do not recommend a different specialty letter for an application, the program directors always fear being a second option. So it doesn't matter what specialty LOR is attached, but the LOR should either state interest in the specialty that you are applying OR it should state a general interest in medicine. Do NOT upload a LOR to a specialty that states a different interest than where you are applying.
