Illinois License-CT form..Plz Help!! - v2k8

Hi, I am from Andhra Pradesh, India... I want some help regarding the completion of Ct form by the Medical Council... How does the process work??

Whom do we have to meet in the office and are there any documents to be submitted..

Plz can anyone throw light on this..

Go to the licensing agency with your documents...they will fill out the form and either mail it or return it to you...

thanks wolverine9 , anyone has experience with the MCI New Delhi filling this form . do they fill it right like the IL state wants it ?

v2k8, go to the state medical council office in your city, in the state of AP.
I filled the top half and they filled the rest of it for me.
I just took copies of my marksheets, copy of my mbbs diploma, and my license certificate.

I am not sure if you have to pay.. every state medical council will be different i guess.
I had to pay fees to renew my license as it had expired.

rinnu, no it clearly states on the form that you need to get a verification from any state or any country that you have had a license, you need the CT form signed.
