q from some other forum - hhh

40 yo man has sudden onset of excruciating head and neck pain while carrying books up stairs.
Temp 98.6, BP 130/90 and pulse 90/min.
Has photophobia and develops eye pain with lateral eye movements.
Neck is markedly stiff and cannot be passively flexed.
Has diffuse hyperreflexia an all extremities with normal strenght and sensation.
Plantar reflexes are flexor bilateraly.
Diagnosis?

1. Cervical osteoarthritis
2. Meningitis
3. ruptured cervical disc
4. subarachnoid hemmorage
5. syringomyelia

Since the symtomes are sudden when carrying books , i go with 3. ruptured cerical disc

I go for Three as it is sudden not very febrile

seems pretty straightforward, i'd go with SAH (5).

eye symptoms, hyperreflexia, neck stiffness, sudden onset.
