A 64 year old man is brought to see you by his wife because of increasing memory problems and confusion over three months. He has become gradually more Iistless and apathetic, with a significant change in personality. She has noted that he has had increasing difficulty with recalling recent events. The patient does not complain of memory problems, but has complained of bilateral
headache over the past two months. He has drunk four bottles of beer every day for twenty years~ although his drinking has decreased over the Iast two months. His gait is slightly unsteady and wide=based. What is the most Iikely diagnosis?
a) Chronic bilateral subdural hematoma
b) Glioma involving both frontal Iobes
c) Azheimer's disease
d) Hypothyroidism
e) Lewy=body dementia

chronic alcohol intake and pt's predispose towards chronic subdural hematoma which can explain her severe headache and gait abnormality.

lewy body dementia usually present with earlier onset of dementia, halluniosis, and frequent fall which doesnt fit in this case.
though personality does changes in some glioma's of frontoparietal region but it has one more common presentation that is seizure and our patient doesnt have any Hx of seizure.

alzhiemer's have ore chronic presentation n dementia,
hypothyroidism may cause apathy and mild mental status alteration but headache and gait disturbance doesnt fit.
