Observership in H4 - docsh

Hi,

I am in h4 visa and I have cleared step1.

I have read many posts regarding this and some say that we can do without pay and some not at all.
So my question is, is it really necessary to do observership ?
Do MS/MD programs has stringent requirements for observership ?
In case I do observership at some private clinic, will they provide me experience letter and will it really help me in my application ?
