A 33 year old female comes to the office for regular exam. Her last menstrual period started 10 days ago. You did an ultrasound and there is a 4 cm, single, loculated, fluid filled adenexal mass. Next best step? Patient is asymptomatic
1. Repeat the US in 2-3 months
2. aspirate the cyst
3. order CA 125
4. Order alpha feto protien
5. prescribe OCP

1 asymp patient, just 10 days in her cycle, I will wait for it to go away on its on

the confusing part is last menstrual cycle started 10 days ago. I thought she is having mennorhagia. They could have said, her LMP was 10 days ago. Isnt that confusing if she is still bleeding, then we should prescribe OCP?

hmmmmmm I didnt read it like that because the question said she came for a regular visit. Not that she was complaining of bleeding. Some women do bleed 7-10 days even though last 3-4 days are very scant bleeding although average bleed is 7 days It is saying she is asymptomatic....so in my mind she is not still bleeding. I can be wrong so I am interested to see what other people think.
