ostrogen supplementation causes increased SHBG - INCREASED total testosterone.DECREASED FREE TESTOSTERONE.Decreased free testosterone will lead to decreased libido.So ill go with AA.

Another cause is because of surgical menopause(TAH-BSO), all hormones that produced from ovaries are decreased.
Oestrogen replacement replish the estrogen and thus why pelvis is well oestrogenized, but testosterone level is still low whichl is the major role of libido.
