also I forgot to tell you that almost all Ortho sponcer only J1 .. which is not so bad and actually it is the only way to go if you are applying for surgery in general because H1 is only for 6 years while J1 limite is your program length

Cleveland Clinic is #5 in the country for Orthopaedics. Don't know what your source is "hard".

1. Mayo Clinic, Rochester, Minn.
2. Hospital for Special Surgery, New York
3. Massachusetts General Hospital, Boston
4. Johns Hopkins Hospital, Baltimore
5. Cleveland Clinic

Cleveland Clinic is a hospital, Case Western is a University with its own hospitals.
In no way case Western is better than the Cleveland Clinic for IM. What is your source ?
To learn more about the merging of the two, read here :

best hospital is not the same as best program
you have to look at the factors they use to do the rankings
having 3 nurse to patient ratio does not tell you they are teaching their residents well
the best way to rank a program is its "assesment score by residency directors "
in the same usnews.com you have to pay for the book to get the entire list
the clinic just started their med school how commited you think they were to teaching
dont get me wrong the clinic is still amongst the best programs in america
and img or amg would be lucky to go there
