NBME 3 .... carotid stenosis - csfirst

A 70yo woman with single episode of TIA. She has DM type 2 controlled with diet. Seven years ago, she had lumpectomy & radiation for localized breast cancer and has had no evidence of recurrence. P/E shows a left carotid bruit. Arteriography shows 80%-90% stenosis of left internal carotid artery. On admission her glucose level is 310. After adequate hydration her glucose is 180. Electrolytes are normal. Which of the following is the most appropriate recommendation to prevent cerebral infarction in this patient?
A) Endarterectomy + daily aspirin
B) Endarterectomy + low-dose heparin
C) Endarterectomy + warfarin

Please explain why A is correct and not C.

HERE CHADS score is 1 so only asprin....
if score wud hace been greater than 2 then we needed to add an anticoagulant...

So according to that table, his score would be 1 (diabetes) + 2 (TIA episode) = 3 ????
