Using various sources from the forum, I tried to create my own little table with mnemonics for CCS.
First column- Just make a note of these things Location Demographics Important points in H/O Second column-DD Third column-Emergency orders POLICE-S Pulse oximetry Oxygen IV Access IV Fluids Cardiac monitor EKG,Elevate Head end Sugar-blood glucose If needed MONAA Morphine Oxygen Nitrates Aspirin Antidotes in over doses,Thiamine, Naloxone Fourth Column-Management 1.Physical Exam 2.CBC LFTs ICU PAX CBC BMP Cardiac enzymes LFT, Lipid panel (as LFT is the second word in mnemonic, L stands for two things) FOBT Thyroid tests Stool tests I-Immunological tests(ab, antigens,ELISA,Wes Blot,PCR, tumor markers,etc) - Iron studies,reticulocytes,LDH,Haptoglobulin -Imaging(U/S,CT,MRI,ECHO,Scopes) (ICU is third word in mnemonic, and 'I' stands for three things) C- Cultures(blood,CSF,joint fluid,sputum, wound swab,etc) U-Urine analysis, culture, urine toxicology P- PT-PTT - Pregnancy test - Pap smear - Pulmonary tests(PEFR,FEV,V/Q,D-dimer,PFT, ABG) (PAX is fourth word in mnemonic, so P stands for 4 things) A-Amylase, lipase X- X rays 3.CCC- Comfort- treat pain, fever, nausea, constipation Consult Cure- medications, procedures 4.If admitted PANIC-R PPI, Activity, NPO, Input output,Compression stockings-Repeat needed tests like CBC, BMP,PT 5. If procedure NBC PIP NPO,Blood group and type,Consent,PT,INR,PTT. This helps me to not miss basic stuff and is like a check list for CCS. Last column- Last screen AFRAID BEAMSSS A - in adults Follow up Reassurance Advance directive Immunization Diet, Drug abuse Bracelet, seat belt Exercise Alcohol Medication-compliance,s/e Smoking Sex Screening In Kids RSVP R-Reassure parent S- Safety V- Vaccinations P- Parent education
