Hi I am doing an application and there is a space for influenza vaccination

influenza vaccination [ date ? , lot# ? , manufacturer ? ]

I am not familiar with this vaccine in my country .. can u give me some examples of .. influenza vaccination [ date ? , lot# ? , manufacturer ? ]

especially the Lot# .. what does this mean ?

whenever u give a vaccination by MD or Nurse in hospital or in clinic, lot # and manufacturer are recorded, they are written on the vial.
