Should i give her triple therapy or zidovudine only?

If HIV and already on medication, continue the medication. If newly diagnosed, give medication as a non pregnant except efavirenz due to teratogenicity.

If the mother has low CD4 (below 500) or high viral count (greater than 100 000) or Opportinitistic infection is present: GIVE TRIPLE THERAPY( for mother and baby safety).

If not: zidovudine ONLY (for baby safety).

look at gestational age , cd4 count, viral load....

if cd4 count 1000 then start therapy irrespective of gestational age with triple therapy.

if she is already on therapy and hiv+ then continue therapy but stop efavirenz.
