Following recommended Immunization Schedule, this girl should received Td ((Diphteria tetanus toxoid) at 11-12 years of age and then every 10 years.
This girl is 16 years of age and she received DTaP, Hepatitis B, MMR, Oral polio until. 5 years of age, so she just need to receive Td

Why she needs a booster of DTaP?
She received I dose of DTap at 2 months, II dose at 4 months, III dose at 6 months, IV dose between 15-18 months and V dose between 4-6 years of age. In Q mention that She has not had any immunizations since the age of 5 years so probably she received all 5. doses of DTap and she just need Td which is given at 11-12 years.
