We are all(mostly) hoping to get into residency so will not know about taxes, refunds of working doctors.

if you are a fresh off the plane/boat, you may have a tax treaty with the parent country which may place you at a different taxable tier. If you are a US resident for tax purposes then you will pay tax like any other. If you are filing a single person with standard deductions you will get some money back as refund, but it all depends on case by case basis.

as a doctor when your earning goes up, you will be taxed accordingly, your returns may change.
