95/94 certified, 2 months usce, 4 LOR, 1 1/2 years of residency in my country.

Can you tell me how Cook County offered you a prematch? By e-mail, phone call ?
What did they ask you? or how did they deal with you while you were making decision?

i believe Cook County is better, but in terms of fellowships there is a lot of competition and if you need H1 that also make it worse bec fellowship programs usually do not sponsor ..and also cook county a litte busy program ..I do not know so much about St.Francis ..
you have very good credentials, how many interview you got? Why don't you go match if you have an university hospital call?
anyways good luck in residency whichever you choose..

Thanks for all the responses, I haven´t decided if I´m going to accept it because I had 2 calls from university programs. The problem is that I´m afraid that going for the match would be too risky for me because my english isn´t great. I think that this may diminish my chances. I got the offer by phone call.

i would choose illinois... cook county is a county hospital.. workload is heavy...
