I mean apart from the risk of having to go back, is there any other restrictions or other problems that one may have while doing the residency or fellowship.

with a J1 visa u will have to work in an underserved area(waiver job) aftert completeing ur residency to switch to H1/green card.waiver jobs r usually in very small towns in the US.that is the only disadvantage.and if u can't find a waier(which is rare) then u have to travel back to ur country.
finding a waiver job isn't a prob...it's just that the areas that qualify for 'underserved' r in remote states like Maine, new mexico...so u shud think abt that!
