There is no fee for the externship, it is free, but they do not pay you either.
You work at the level of resident, malpractice is covered by them.
For application form email the PC and ask the form to be sent to you for next application cycle.
They take 6-8 externs, depending, but they might "fire" you too if you do something stupid.
