If its any consolation, there are thousands of doctors like you here in the states that have been caught up with not getting into residency on time; I call it the eras tease. Every year it looks like it will be better, you do an observership, reapply and it doesn't work.
Before you know it, you have spent almost the ame number of years you were in med school pursuing residency.
To answer your question, it you get into residency with or without step 3, ideally the hospital obtains a provisional license for you, ie they hedge there bets and you can practice as long as you are under their wings. Once you are done then you are on your own.
