I am an IMG. My credentials are 241, 238, pass in step 1, 2 & cs respectively.

I have more than 4 LORs from US Physicians viz. Cardiology (2), Hematlogy and Oncology (1), Pulmonary and Critical Care (1), Interventional Radiology (1), EM (1).

Since none of the above are from IM per se, I wanted to know whether I should use the letter from my HOD from my medical school (Nepal) or should I use all US physician LOR.

Use all the US ones, especially the IM subspecs.

Thanks for your advice.

i am not sure where i picked this from but I heard that one letter should be from the HOD. I will have to got thorough a lot of pains in acquring one. The one that I have in my posession is dated 2011 and does not have my AAMC id.

I wanna know if I can take the risk of not arranging for a current one at all.

My concern comes from the fact that none of my letters are from IM.

Your letters are from IM, they are from IM sub specialties which still count.
some programs do require a letter from the department chair, so it wont hurt for you to get a letter from the HOD and file that in with ERAS. And research your programs individually and submit the letter of the HOD to these specific programs. Otherwise I would just use your US LoR.
